Community Care Alliance is proud to offer an array of services supporting individuals who are homeless/unstably housed and coping with mental health and substance use challenges. Many of these services are part of Safe Haven. The Integrated Health Home team is comprised of a multidisciplinary team. We are searching for a person-centered, trauma-informed Clinician to support this team. If you thrive in an environment where every day offers a unique new challenge, our team is for you. Conduct comprehensive biopsychosocial assessments, assist with Intake, and provide counseling in both individual and group settings. Provide education to the team regarding mental health and substance use issues. Participate in case reviews. Conduct risk assessment and connect individuals with higher levels of care. Requires excellent verbal and written communication skills. Demonstrate competency in CBT, DBT, and MI. Skilled at developing treatment plans and using an EHR.
